Updated Course Evaluation Form

We are in the process of updating our core course/clerkship evaluation form to match the recently updated faculty evaluation forms. This form shares the same design principals:

• Required question(s) are at the top, followed by a single text entry area for comments.
• Optional questions default to “No Opinion” to minimize the number of clicks required.
• Custom questions (if any) will appear below the core questions shown here.

Questions specific to the course director have been moved to a new faculty evaluation form specific to this role.

Not shown is a question that addresses the eighty hour rule for clerkships only.
